Do I need Health Insurance in Germany ?

German law requires all citizen living in Germany to have health insurance.

(*) If you are from a member of the Schengen Agreement or a country from Japan, Korea, Australia, Canada, Israel, New Zealand, Switzerland or the United States and stay within 90 days without a visa, you will be exempt from purchasing health insurance.

The timing of applying for health insurance

If you are not insured health insurance , you will not be able to apply for any visa.

Therefore, if you have to apply for a visa, you must apply for health insurance before applying for a visa.

Public Health Insurance

There are several public insurance companies in Germany.

If you have German public insurance, you will not be charged for medical examination and treatment even if you receive a medical examination or treatment at a hospital most of the time.

The insurance premiums for every month covers 100% of medical examination and treatment costs.

And there is a system doctors and hospitals charge medical fee directly to public health insurance companies.

However, accident insurance and personal liability insurance are not included, so it is necessary to purchase additional insurance if you need. Also, some medical expenses such as aesthetic dentistry are not covered so you have to purchase additionally.

People who can obtain public insurance are limited to those who have a valid reason to stay in Germany as follows:

  • Employees working for German companies
  • Freelancer and Self-employees
  • Students and exchange students studying at German universities
  • PhD students, internship students
  • Refugees

However, the following people cannot apply for public insurance even if they have a valid reason to stay in Germany:

  • Language school students
  • People in working holiday
  • Au pair students
  • Master and Bachelor students from abroad who is older than 30 years old
  • PhD students from abroad and receive any scholarshop
  • People who do not have a visa to stay for more than 366 days

These people have to apply for private insurance.

Private Health Insurance

The company that sells private insurance is a private company, and unlike public insurance, insurance premiums can be freely determined by the company.

There is no cheap and good private insurance because premiums and coverage are proportional. However, there are private insurances with expensive premiums and poor coverages. There are a variety of private insurance policies in Germany, so compare premiums and coverages to find a good one.

The difference between public and private insurance

  • If you have public health insurance, you can change your insurance company at any time after 18 months. However, if you have private health insurance, you may be refused by a private health insurance company.
  • Public health insurance premiums are determined based on income, and private health insurance premiums are often set based on age.
  • Changing from public insurance to private insurance is difficult, but changing from private insurance to public insurance is easier.
  • If you receive treatment with private insurance, you pay your doctor first. Later, you bill an insurance company.

Students studying at German universities can basically apply for public insurance.
We basically recommend applying for public insurance because most of universities in Germany does not accept private health insurance.

Also, you should apply for public one first.
Since once you choose private insurance, it is difficult to change to public insurance.

However, only the following people can apply for public health insurance:

  1. Students who are under 30 years old and have not completed more 14 semesters
  2. Exchange student

People who doesn't apply to any of them have to apply for private health insurance.

You can choose between private and public insurance. However, if you have never had any health insurance in Germany or schengen area before, there is some rules to have a public health insurance.

PhD students have no right to apply for public health insurance when the following applies:

  1. You receive any scholarships
  2. Older than 30 years old
  3. You have finihsed more than 14 semesters
  4. You are from a country that is not in the EU and has no social security agreement with Germany
